Strengthening Patient Safety in Radiation Oncology: A Nursing-Led Protocol for Monitoring Patients with Cardiovascular Implantable Electronic Devices

Abstract
Patients with cardiovascular implantable electronic devices (CIEDs) undergoing radiation therapy are at increased risk for device malfunction. Inconsistent communication, unclear role delineation, and lack of standardized procedures contribute to gaps in monitoring and follow-up, increasing the potential for patient safety events. Evidence-based guidelines recommend structured workflows, interdisciplinary collaboration, and standardized monitoring for this population. The purpose of this DNP scholarly project was to develop and implement a nursing-led, evidence-based protocol to standardize the identification, monitoring, and follow-up of patients with CIEDs receiving radiation therapy, thereby improving safety and interdisciplinary communication. The project utilized the Iowa Model of Evidence-Based Practice and a phased implementation strategy. Interventions included staff education, policy integration, role clarification, and development of tracking tools. Data were collected through chart audits, completion certificates, and pre-/post-education surveys using Microsoft Forms. Outcomes measured included nurse education completion, patient education, adherence to risk stratification and monitoring protocols, and post-treatment cardiology follow-up. All nurses (100%) completed the Heart Rhythm Society education and post-assessment. All patients (n = 7) received education, were correctly risk-stratified, and achieved full adherence to the monitoring protocol and 71% completed follow-up with their cardiologist. Pre-/post-surveys showed an increase in nurse confidence and preparedness regarding CIED patient management. This initiative demonstrated that a nurse-led, evidence-based protocol can significantly improve standardization and interdisciplinary collaboration in the care of CIED patients. Sustaining and expanding this model across network locations can further enhance safety, communication, and consistency in oncology nursing practice.
