Description
This is a sample Attendance Accommodation Agreement form to be completed by the student, faculty member, and DSP. Some sample language has been provided to reflect an interactive process of determining how attendance accommodations may apply in individual courses. Review this language in relation to your campuses attendance policies and adjust accordingly.
