The purpose of this literature review was to identify if skin breakdown assessment tools are effective in predicting whether or not patients are at risk for developing pressure ulcers while hospitalized. A comprehensive review of four studies was completed and revealed that a lower score on a skin breakdown risk assessment tool was directly correlated with increased length of stay and that tools must be paired with interventions in order to decrease risk of developing a pressure ulcer. A tool is only good to identify risk, and will not decrease risk without intervention. The results also reveal that training of all staff on the usage of risk assessment tools is vital in yielding accurate results.
