This form is an example of an agreement to be completed by students with a granted Emotional Support Animal accommodation. Included is some recommended language for the rights and responsibilities of living on campus with an Emotional Support Animal. The form allows requires that students provide emergency contact information for someone who can provide care for the animal if the student is unable to do so. Please review your campuses Emotional Support Animal Policy and adjust accordingly.Citation
