Abstract
The risk of maternal infection from labor and delivery is higher for mothers who deliver via cesarean section (C/S) versus vaginal birth. However, there is no clear recommendation for skin preparation to reduce morbidity in patients after C/S. Each institution has different policies about pre-operative C/S abdominal prep. While most institutions implement an abdominal iodine paint immediately prior to the surgery, many hospitals differ in their protocol regarding another abdominal prep beyond the iodine paint. The purpose of this project was to determine whether using a pre-operative chlorhexidine abdominal wipe prep in addition to the surgical iodine paint prep will lead to a decreased incidence of infection. A search of the literature using the key words of abdominal prep or skin prep and cesarean section or c-section or caesarean or labor/laboring or L&D revealed a small number of relevant articles, many of which were from greater than 10 years ago. Furthermore, few articles directly compared the superiority of a chlorhexidine abdominal wipe prep in addition to the iodine paint in decreasing the incidence of infection. Here, 1 systematic review and 3 articles were analyzed. Within these articles, there was no consistent conclusion confirming or refuting a decreased incidence of post-cesarean section infection using a pre-operative chlorhexidine abdominal wipe in addition to the surgical iodine paint. Further research is necessary to not only compare one agent of abdominal skin preparation to another but also to investigate the value of adding an abdominal wipe prep prior to the iodine paint for pre-operative cesarean section.
