Abstract
Readmission to the hospital within 30 days of discharge has been linked to poor standards of care and quality performance issues. Stroke is in the top 10 causes of readmission among Medicare and Medicaid patients. The creation of a concise and standard system for continuity of care and education would make impact on these figures. Establishing a process for transitions of care, addressing the opportunities for change from a multi-disciplinary aspect along with assessing and documenting the education tools and comprehension of patients could reduce these instances of preventable readmission.
