Abstract
The purpose of this literature review was to analyze how different care bundles can reduce readmission rates in Chronic Obstructive Pulmonary Disease (COPD) patients. A care bundle is a set of evidence based practices aimed at improving patient outcomes after discharge from acute care settings following an exacerbation (Ospina et al 2016). Under current federal guidelines hospitals are required to cover medical costs if patients are readmitted within thirty days of discharge from an acute care setting. According to the Healthcare Cost and Utilization Project in 2013 COPD patients had a 20% 30 day readmission rate which costs about 1.4 billion dollars.
